Trinity Cemetery Memorial Tree Donation Form

As a part of the Trinity Cemetery Restoration project, we will be planting 3 Pyramidal Cedars, 3 Columnar Oaks, and 5 Rose of Sharon within the cemetery for a total of 11 trees. If you would like a tree to be memorialized for a loved one please fill out the form below.

Cost per memorial is based on the tree type listed below.

THIS FORM IS NOT A CONTRACT but rather an indication of interest. A representative of the Trinity Cemetery Committee will contact you for further discussion regarding the memorialization.
